Good Faith Estimate
Good Faith Estimate
Effective Date: August 9, 2026
Camellia Wellness is a self-pay mental health practice. We do not bill insurance companies for our services.
Under the federal No Surprises Act, you have the right to receive a Good Faith Estimate of the expected cost of your care.
Your Right to an Estimate
Before you receive care, you may ask Camellia Wellness for a written estimate of your expected charges.
When required by law, Camellia Wellness will provide a written Good Faith Estimate when you schedule services in advance.
Your estimate will be based on the services that are reasonably expected at the time the estimate is prepared.
If your care or requested services change, your actual charges may be different from the original estimate.
Our Fees
Our current fees are listed on our Services & Fees page.
A personalized Good Faith Estimate will be provided through the practice's scheduling or patient portal process when required.
Please keep a copy of your Good Faith Estimate.
